Full Service Provider Credentialing Annual Subscription Package -
For an annual fee you will receive a dedicated credentialing staff that will take care of managing all your
credentialing documents and will be your primary point of contact for any credentialing matters that may
arise in your practice. Some of the features of our credentialing subscription include:
*re-credentialing applications received prior to your subscription effective date or within 90 days of your subscription
effective date will be charged the new network application rates.
New Practice Enrollment Package -
If you are starting a new practice, we offer a package that allows you to get the key credentialing needs
taken care of for your new practice and one provider. This package includes completion of applications
for a Medicare Supplier/Group number (application for your entity provider number), one individual
provider Medicare application, one individual provider state Medicaid application, and completion of
an individual provider CAQH* profile. In addition to the enrollment services described above, you’ll
receive up to 5 hours of credentialing consulting services to assist with other credentialing or contracting
needs that may arise as you start your new practice. *If your provider(s) already has a CAQH profile, we
will review & update his/her profile as part of this package.
New Provider Enrollment Package -
If you have a new provider joining your group, this package will provide you with three complete
applications to get you started. This bundle of service includes completion of applications for an
individual provider for Medicare and state Medicaid, and completion of the CAQH* provider profile or
two commercial carriers of your choice. *If your provider(s) already has a CAQH profile, we will review &
update his/her profile as part of this package.
Basic Service Package: Per Application -
If you don't want a subscription or bundled service product then we can assist you on a per application
basis. With our basic service, you simply tell us what insurance companies or hospitals that you want to
enroll with and we’ll complete all the forms on your behalf, submit them to the carriers or hospitals and
follow-up until the application is processed. This service includes:

- Flat Rate Plan - This plan is available for our contracting, credentialing & state licensure services.
See additional details below or contact our office for a quote! - Hourly Plan - This plan would be chosen when assistance is needed only one time or just a few hours
a week or per month. Due to the individuality of each client, prices vary according to each project
and are tailor made to suit each client or individual.

participate with CAQH (after you have a completed CAQH profile) - Complete re-credentialing applications that you receive during your subscription period*
- Updates to Medicare credentialing records as needed
- Providing quarterly updates to your CAQH file so that your participation with commercial carriers is
uninterrupted - Serve as your primary point of contact for credentialing issues for insurance companies and
hospitals - Credentialing staff with extensive experience in working with Medicare, Medicaid, and
commercial insurance enrollment

Payment Information -
We accept payment by check, wire transfer, PayPal and we also accept Visa,
Mastercard and American Express. Please note however, that a convenience
charge of 3.5% will be added to any credit card transactions. To pay by credit
card, please download our Credit Card Authorization Form and once you fill it out,
